Can I Manage the Transition to Aged Care Journey Myself?

The transition to residential aged care journey is complex (requiring concurrent management of many elements), confusing and time consuming. ConciAge is here for those who want a professional approach that will free up their time and provide a better outcome.  We reduce anxiety, and leave you time to direct energies to emotional support rather than becoming lost in the quagmire of the practicalities.

Where does ConciAge Operate?

ConciAge currently operates in the Sydney metropolitan area, the Central Coast, North Coast, Blue Mountains, Illawarra, Shoalhaven, Southern Highlands. Services in Melbourne, regional Victoria, Brisbane and regional Queensland and services nationally are planned soon.

I am Overseas / Interstate is ConciAge an Option?

ConciAge can assist families whether interstate, overseas or simply unable to meet because of other commitments.  Often it is impractical to arrange the time required to ensure a seamless move and impossible to do this remotely.  ConciAge manages for you.  We offer a number of video or call conferencing options,   accommodate after hours or multiple time zone communications and provide regular email updates.

My family is Large / Complex - What Can We Do?

When there are multiple family members involved ConciAge allows the sharing of responsibility by managing the process rather than only one or a few members taking on what is a substantial undertaking.   Where there is family conflict we can offer options  for clarifying and reaching agreement on an approach, so that focus returns to the person who undertaking the residential aged care journey. 

Is ConciAge Available Outside standard Working Hours?

ConciAge is a journey based service and our objective is to be available when and as required.  Our Consultants will make appointments to suit you including outside standard working hours.

There may be Issues of Mental Capacity?

ConciAge has an excellent understanding of capacity issues relating to decision making, for example dementia, and will work with you to optimise outcomes.

How do you Work with Physical or Other Special Needs?

Each person’s circumstances are different, ConciAge’s will tailor any of services to work for you or design new services to meet your needs.

Do you Understand LGBT+ Requirements are Sometimes Different?

ConciAge has a real understanding that sometimes GLBTIQ clients  sometimes have different requirements and concerns and will work closely with you to understand and address these.

Our Preferred Language is Not English - Are Translators Available?

If you or your family member prefers to communicate in a language other than English we will find Consultant who speaks your language or engage a translator for you.
